Providence CARE Counselors work diligently to enroll qualified applicants in financial assistance programs and connect applicants with needed community resources.

Most applications require the following information:

  • Proof of ID
  • Social Security number
  • Birth date
  • Pay stubs, W-2 forms or wage & tax statements
  • Policy numbers for any current health insurance
  • Information about any health insurance you or your family could get from your jobs
  • Household income info
